Employment Verification

There may be times when persons outside of the Agency may call requesting information about your employment with us. All information, other than job title and dates of employment, will only be released upon your written authorization. Should you be applying for credit or a mortgage, you must notify the Human Resources Department in writing as to what information you authorize to be released and to whom.

The only other time that information will be released about your employment with the Agency is to comply with a court order or subpoena.

Change in Personal Status

During employment, the Agency must maintain personal information on each employee. Such information includes martial status, number of dependents, home address and telephone number, etc. This information helps us to ensure that you are paid properly and that appropriate taxes are deducted from your paycheck. Please let your supervisor or the Human Resources Department know of any changes to this personal information.

Access to Your Personnel File

The Human Resources Department is responsible for maintaining an individual personnel file and a separate medical file on each Agency employee. You may review these files by simply making an appointment with the Human Resources Department. A space will be provided for your privacy. Copies of any document(s) can be made upon request. Personnel and medical files are not permitted to leave the Human Resources Department.

Your supervisor is authorized to review your personnel file, but not your medical file.

Leaving the Agency

It is the policy of the Agency that either the employee or the employer can terminate the employment relationship at will; however, termination of employment will not be initiated without prior review of your supervisor or an appropriate member of management.

We hope that your employment with the Agency will be rewarding. However, if you decide to resign for any reason, we would appreciate your giving us at least two weeks’ written notice, so we may provide adequate service to our clients.

When you leave the Company, whether you resign or are discharged, you will be paid any unused vacation time you have previously earned. This payment will be included in your final paycheck. Final paychecks will be available no later than the next regular pay day and will be mailed directly to your home.
